Travel Information

Vaccinations for travel are available at the surgery. Please enquire at reception regarding vaccinations or complete and return the Travel Risk Assessment Form below. Please note we require at least SIX WEEKS NOTICE PRIOR TO TRAVELLING to ensure we can offer you a suitable appointment should you need one, as our nurse appointments tend to get booked up in advance, and to give enough time for our nurses to check what vaccinations and advice you require prior to your appointment.

The Travel Risk Assessment Form can be handed in at reception or emailed to [email protected]  Once we have received the completed form, a member of the Practice will contact you to advise if any vaccinations are recommended and offer you an appointment with our Practice Nurses if required.

Please note there may be a charge for some vaccinations. We will advise you of this at the time of offering you an appointment.

Coronavirus (COVID-19)

Stay at home for 7 days if you have either:
– A high temperature – you feel hot to touch on your chest or back
– A new, continuous cough – this means you’ve started coughing repeatedly
Do not go to a GP surgery, pharmacy or hospital.
You do not need to contact 111 to tell them you’re staying at home.
Testing for coronavirus is not needed if you’re staying at home.
