If you’re planning to travel outside the UK, you may need to be vaccinated against some of the serious diseases found in other parts of the world.
Please complete our ‘Travel Health Questionnaire’. The nurses will look at the health questionnaire to see if you need any vaccinations. Please note there may be a charge for some vaccinations. We will advise you of this at the time of offering you an appointment.
The ‘Travel Health Questionnaire’ can be handed in at reception or emailed to [email protected]. The surgery will then contact you to arrange an appointment if necessary.
- Please note we require at least six weeks notice prior to travelling to ensure we can offer you a suitable appointment should you need one
